Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing Facility (Cumberland, WI)

We’re expanding our team that provides patient-centered primary care in skilled nursing facilities around Cumberland, WI. This full-time opportunity offers a flexible schedule with no overnight shifts or on-call requirements. You’ll work alongside physicians and facility staff to deliver high-quality, evidence-based care to residents in a collaborative, supportive environment.

What the schedule and role look like

• Full-time and part-time options with flexible scheduling.
• Typical shifts are six hours and rounds usually start between 7:00 a.m. and 10:00 a.m.
• No call or overnight responsibilities.
• Average caseload of about 15-20 patient visits per day.
• Daily rounding, managing both acute and chronic conditions, prescribing medications, and developing individualized treatment plans.
• Close collaboration with nursing staff and interdisciplinary teams to coordinate care.
• Responsibility for maintaining clinical and regulatory compliance.
• Company-provided iPad with LTE for charting and communication.

What we’re looking for

• Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license.
• NPs: board certification through ANCC, AANPCB, PNCB, or AACN. PAs: NCCPA certification.
• Board certified or board eligible in FM, IM, Geriatrics, PM&R, or Emergency Medicine is preferred.
• Strong clinical judgment, excellent communication skills, and a patient-centered approach to care.
• Able to work independently while collaborating effectively within a multidisciplinary team.
