Adult Geriatric Nurse Practitioner (Full-Time) - Mesa, AZ | Competitive Pay & Growth

Full-time position in Mesa, AZ providing comprehensive primary care to adult and geriatric patients. Manage acute and chronic conditions, perform assessments, develop and implement treatment plans, prescribe medications, order and interpret tests, coordinate referrals, document in the EHR, and provide patient education with cultural sensitivity and a focus on patient-centered care.

Key responsibilities:

• Conduct histories, physical exams, diagnostic evaluations, and follow-up visits for adults and older adults
• Diagnose and manage acute and chronic illnesses; initiate and adjust treatment plans and medications
• Order, interpret, and act on laboratory and diagnostic tests
• Provide preventive care, health maintenance, and patient education
• Coordinate care and referrals with specialists and community resources
• Maintain accurate, timely documentation in the electronic health record
• Work independently within scope of practice and collaborate with interdisciplinary team
• Participate in quality improvement and continuing education activities

Required qualifications and certifications:

• Graduate of an accredited Nurse Practitioner program
• Adult-Gerontology or Family NP preparation/experience treating adults and geriatrics
• Nurse Practitioner certificate issued by the Arizona Board of Nursing
• Valid Arizona RN license and Arizona Advanced Practice license
• Valid DEA registration
• Current Basic Life Support (BLS) and CPR/AED certification (AHA or Red Cross)
• Level One Fingerprint Clearance Card (required for behavioral health settings)
• Valid Arizona driver’s license

Experience and skills:

• Clinical experience managing adults with multiple chronic conditions and acute presentations
• Strong clinical judgment and ability to work autonomously
• Effective interpersonal and communication skills with patients, families, and team members
• Basic proficiency in business English, spelling, and punctuation
• Comfortable using electronic health record systems and basic computer applications
• Commitment to culturally competent, patient-centered care and ongoing professional development
