• Hybrid role: approximately 50% direct patient care and 50% clinical leadership
• Monday - Friday, 8:30 am - 5 pm schedule
• Full-time or part-time options available; minimum 3 days per week
• Care delivered via telemedicine and in-home visits using a value-based care model
• Lead and supervise a team of Nurse Practitioners as Medical Director
• Focus areas include symptom management, goals-of-care, and advance care planning
• Active California license and board-certified status required; DEA required
• Experience in palliative care or clinical leadership strongly preferred
