Job Description
- Mon - Fri, 8 am - 5 pm -- 8hr shifts, guaranteed 8 hrs/day
- 4 - 6 patients per shift
- In-clinic and community-based visits -- home, ALF, and SNF settings
- PACE program -- exclusively serves adults over age 55
- 11-person interdisciplinary team supports provider
- Geriatric experience required -- majority of population is geriatric
- Full-time preferred -- minimum 2 weeks per month required
- 3 - 4 days per week schedule considered
- Paid malpractice insurance; pre-paid travel and housing expenses
- Assignment details and time entry in online portal
