This opportunity is for an experienced Nurse Practitioner or Physician Assistant to provide comprehensive primary care for children in a mission-driven, value-based care setting focused on improving outcomes for underserved populations. The role includes well-child visits, sick visits, chronic disease management, preventive care, and close collaboration with families and care teams, with a patient volume of approximately 25-30 patients per day over a four-day clinical schedule. Candidates should have at least one year of pediatric primary care experience, with community health experience and Spanish language skills viewed favorably. • Permanent full-time position
• Nurse Practitioner - PNP or FNP / Physician Assistant
• Must have Pediatric Primary Care experience - 1-2 years for NPs and 4-5 years for PAs
• Mon-Fri - 4 Days in Clinic (Cover 1 Saturday/ month)
• 25-30 PPD (Well visits, Sick visits, and some Walk-in) No Procedures
