Job Description
Provide comprehensive, patient-centered primary care in Buckeye, Arizona. In this full-time role you will evaluate, diagnose, treat, and manage patients across the lifespan, emphasizing prevention, education, and care coordination. Work independently within your scope, follow clinic policies, and collaborate with the interdisciplinary team on routine and complex cases.
Key responsibilities:
- Conduct history and physical exams, order and interpret tests, diagnose and manage acute and chronic conditions
- Develop and implement individualized treatment and preventive care plans
- Prescribe medications within scope and maintain accurate medical records in the electronic medical record system
- Provide patient education, counseling, and follow-up care
- Coordinate referrals and collaborate with behavioral health and specialty services as needed
- Adhere to clinic policies, quality and safety standards, and regulatory requirements
Minimum qualifications:
- Graduation from an accredited NP or PA program
- Current Arizona license and certification to practice as a Nurse Practitioner or Physician Assistant
- Valid DEA registration
- Current BLS/CPR/AED certification for healthcare professionals (AHA or Red Cross-compliant)
- Valid Arizona driver’s license
- Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety (required for specialty behavioral health assignments)
- Demonstrated clinical competence and judgment
- Basic business English, spelling, and punctuation skills
- Basic computer skills, including proficiency with electronic medical records
