• Mon - Fri, 8 am - 5 pm -- weekends and holidays off
• Guaranteed 40 hours per week
• 5 - 6 regular visits per day -- 3 visits on admission days
• Majority home health setting
• 6.5 points per day
• OASIS training preferred -- 1 year of experience preferred
• All ages served -- pediatric cases are infrequent
• Active physical therapy license required
