Adult Nurse Practitioner - Primary Care (Montebello, CA)

We’re looking for an experienced Adult Nurse Practitioner to join our team in Montebello, California and deliver compassionate, patient-centered primary care in ambulatory and community-based settings. This is a chance to focus on preventive health, chronic disease management, and long-term wellness while working closely with an interdisciplinary team.

Responsibilities

• Provide comprehensive primary care to adult patients, including evaluation, diagnosis, and management of acute and chronic conditions.
• Perform preventive care, routine assessments, and health screenings.
• Prescribe and manage medications, monitor treatment response, and adjust plans as needed.
• Create individualized care plans and offer clear patient education to support self-management and healthy behaviors.
• Support long-term disease management through follow-up, care coordination, and referrals.
• Collaborate with physicians, nurses, and other members of the healthcare team to ensure continuity and quality of care.
• Maintain accurate, timely clinical documentation and adhere to clinical protocols and standards.

Compensation & benefits

Requirements

• Master’s or Doctoral degree from an accredited Nurse Practitioner program.
• Current, unrestricted California Nurse Practitioner license.
• National Board Certification as an Adult Nurse Practitioner or equivalent adult-focused primary care certification.
• Current DEA registration or eligibility to obtain one.
• Preferred: experience providing primary care in ambulatory or community-based settings.
• Strong clinical assessment, diagnostic, communication, and patient education skills.
