Family Medicine Physician - Team-Based, Value-Based Care in Chula Vista (10-15 pts/day, Extended Visits)

Join a collaborative, team-based primary care practice in Chula Vista, San Diego, committed to whole-person, preventive medicine. We seek a Board-Certified Family Medicine Physician who values relationship-based care and improving patient outcomes within a value-based model.

What you’ll do

• Provide comprehensive, preventive, and chronic care in extended visits (20-60 minutes) with a manageable panel (10-15 patients/day).
• Work closely with an interdisciplinary clinical team, including health coaches, to coordinate patient-centered care.
• Participate in quality improvement and population health initiatives; leadership opportunities available as the organization grows.

Qualifications

• MD or DO from an accredited medical school
• Completion of an accredited Family Medicine residency (Family Medicine preferred; Internal Medicine considered)
• Current Board Certification (ABFM, ABIM, or AOA)
• Minimum three years’ primary care experience
• Current, unrestricted California medical license (or ability to obtain) and active DEA registration
• Negative TB test within 12 months prior to start date
• Experience in team-based care and with electronic medical records preferred
• Familiarity with medical coding and documentation, including Medicare Advantage and HCC coding
• Comfortable using Google Workspace
