Adult Primary Care Physician - Post-Acute ISNP (Geriatrics) | Las Vegas, NV

Join a growing, patient-centered organization delivering comprehensive care to medically complex Medicare members enrolled in an Institutional Special Needs Plan (ISNP). In this full-time role based in the Las Vegas area, you will provide adult primary care across skilled nursing facilities, long-term care and other institutional settings, emphasizing proactive care management, interdisciplinary collaboration, improved clinical outcomes, and prevention of avoidable hospitalizations.

Key responsibilities

• Provide longitudinal primary care to seniors in SNFs, LTC, and post-acute settings.
• Lead and participate in interdisciplinary care planning to manage complex chronic conditions.
• Implement strategies to reduce hospital readmissions and optimize quality metrics.
• Complete timely documentation consistent with HCC, Medicare Advantage, HEDIS, and STARs requirements.
• Travel between facilities throughout Nevada as needed.

Compensation & benefits

• Base salary range: $211,369-$317,053, plus individual and company performance bonus opportunities.

Qualifications

• MD or DO from an accredited medical school.
• Completion of an accredited residency in Family Medicine, Internal Medicine, or Geriatrics.
• Board certified or board eligible.
• Active Nevada medical license, or ability to obtain licensure; valid DEA registration.
• Preferred: 2+ years’ experience in SNF, LTC, post-acute care, or complex senior care.
• Preferred: Experience with HCC coding, Medicare Advantage, HEDIS, and STARs documentation.
• Comfortable practicing in institutional and post-acute care environments and able to travel between facilities.
