Join a growing, patient-centered organization providing primary care to Institutional Special Needs Plan members in skilled nursing and long-term care settings across Tucson, AZ. Seeking a Board Certified or Board Eligible Family Medicine or Internal Medicine physician to deliver comprehensive, value-based care for geriatric and medically complex patients.

Key Responsibilities

• Provide primary care for SNF, LTC, post-acute, and medically complex patients
• Manage preventive care, chronic disease management, and care coordination
• Participate in multidisciplinary team meetings and collaborate with clinicians, nurses, and care coordinators
• Document care, meet quality metrics, and support value-based outcomes
• Communicate with families and facilities to optimize patient-centered care plans

Compensation & Benefits

• Base salary: $211,369-$317,053
• Sign-on bonus: $30,000

Qualifications

• MD or DO from accredited medical school
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Active Arizona medical license or eligibility to obtain
• Current DEA registration
• Preferred: experience with geriatric, post-acute care, SNF, LTC, or medically complex populations
• Strong clinical judgment, communication, and teamwork skills
• Commitment to compassionate, patient-centered care
