Full-time opportunity in Elkhart, Indiana to provide comprehensive, patient-centered family medicine for all ages. Collaborative practice with multidisciplinary team; additional openings in Michigan City and South Bend, IN.

Key responsibilities

• Deliver primary care across the lifespan: acute care, chronic disease management, preventive care, wellness exams, and screenings
• Establish and maintain long-term patient and family relationships
• Coordinate care with specialists and interdisciplinary teams
• Apply evidence-based medicine to improve outcomes
• Maintain accurate, timely electronic medical record documentation

Requirements

• MD or DO from an accredited medical school
• Board certified or board eligible in Family Medicine
• Completion of an accredited Family Medicine residency
• Eligible for or currently holding an unrestricted Indiana medical license
• DEA certification or eligibility to obtain
• Strong clinical, communication, and interpersonal skills
