Locum Nurse Practitioner or Physician Assistant needed for a value-based primary care practice serving geriatric patients in Brooklyn, NY. Hybrid position combining outpatient clinic coverage and home-based visits.

Compensation

• $75-$95 per hour

Schedule & Workload

• Full time, guaranteed hours
• Monday-Friday, 5 days/week; 8:30 AM start
• Clinic: 7-10 patients/day (small census)
• Home visits: 3-5 patients/day

Setting

• Hybrid: outpatient clinic + home-based care
• Focus: value-based primary care for elderly patients

Key Responsibilities

• Provide comprehensive primary care to geriatric patients: preventive care, chronic disease management, acute care
• Diagnose, treat, prescribe, and develop individualized care plans
• Order and interpret labs and imaging
• Document accurately in EHR
• Educate patients and promote wellness
• Coordinate care with physicians, nurses, and support staff; manage referrals and follow-up
• Maintain compliance with state, federal, and professional regulations

Requirements

• Active NY NP or PA license
• Minimum 2 years primary care experience
• National board certification
• Medicaid and Medicare provider IDs required
• Ability to complete expedited credentialing process as needed
