Locums NP/PA - Hybrid Geriatric Primary Care (Bronx/Brooklyn/Ridgewood) $75-$95/hr

We’re looking for an experienced Nurse Practitioner or Physician Assistant to join a value-based primary care team serving older adults. This locum position blends outpatient clinic work with home-based visits, giving you variety and meaningful continuity with geriatric patients.

• Bronx, Brooklyn, and Ridgewood, NY

Schedule & Workload

• Full-time, Monday-Friday (5 days/week) with an 8:30 am start
• Hours guaranteed
• Hybrid model: outpatient clinic + home visits
• Clinic load: small census (7-10 patients/day)
• Home visits: 3-5 patients/day
• Overall patient panel: low daily census, allowing for focused, patient-centered care

Key Responsibilities

• Provide comprehensive primary care for geriatric patients, including preventive care, chronic disease management, and treatment of acute issues
• Diagnose, prescribe, and develop individualized treatment plans
• Order and interpret diagnostic tests (labs, imaging, etc.)
• Accurately document care in the EHR
• Educate patients and promote wellness and preventive strategies
• Coordinate care with physicians, nurses, and support staff; arrange specialist referrals and manage follow-ups
• Maintain compliance with applicable state, federal, and professional regulations

Requirements

• Active NY NP or PA license
• Minimum 2 years’ experience in primary care
• National board certification
• Medicaid and Medicare provider IDs required
• Expedited credentialing available
• Spanish-speaking clinicians preferred
