Clinical Lead Nurse Practitioner - Adult Primary Care (TeamCare) - Brooklyn, NY

We are seeking an experienced Clinical Lead Nurse Practitioner to join our TeamCare program in Brooklyn, NY. Assigned to a TeamCare site, you will deliver comprehensive adult primary care while serving as the site’s clinical leader - combining direct patient care, care coordination, and leadership responsibilities. In the absence of the Medical Director, or at their direction, you will represent the Medical Director and provide clinical oversight at the location.

Key responsibilities

• Provide high-quality, evidence-based primary care to adult patients.
• Serve as the clinical lead at an assigned TeamCare site, guiding clinical workflows and supporting staff.
• Coordinate patient care across disciplines and settings to optimize outcomes.
• Represent the Medical Director when delegated, including clinical decision-making and staff support.
• Participate in quality improvement initiatives and maintain accurate clinical documentation.
• Collaborate with a multidisciplinary team to ensure continuity of care.

Schedule

• Monday-Friday, 8:30 AM-5:30 PM (1-hour lunch)
• No routine evenings or weekends

Compensation & benefits

• Base salary $162,000-$190,000 (depending on experience) plus bonus

Qualifications

• Graduate of an accredited NP program and eligible for NYSED recognition
• Current, unrestricted NY NP and RN licenses
• AANP or ANCC board certification and active DEA registration
• Current BLS certification
• Active CAQH profile and current Medicare/Medicaid provider IDs
• 2+ years clinical experience in primary care, home care, care management, or related settings; at least 1 year caring for adult patients preferred
• Strong clinical judgment, communication, analytical, and organizational skills
• Proficiency with Microsoft Office and electronic health record systems
• Bilingual skills a plus
