Clinical Lead Nurse Practitioner - Adult Primary Care (TeamCare) - New York, NY

Join our TeamCare program in New York as the Clinical Lead Nurse Practitioner. You will provide comprehensive adult primary care at an assigned site while serving as the on-site clinical leader and care coordinator. In the absence of, or under direction from, the Medical Director, you will represent the Medical Director and provide clinical oversight for the location.

Key responsibilities

• Deliver direct primary care to adult patients, including assessment, diagnosis, treatment, and follow-up.
• Provide clinical leadership and supervision at the assigned TeamCare site.
• Coordinate care across multidisciplinary teams and community resources.
• Serve as clinical liaison to administration and the Medical Director as needed.
• Support quality improvement, clinical workflows, and staff development.
• Maintain accurate medical records and comply with regulatory and billing requirements.

Schedule

• Monday-Friday, 8:30 AM-5:30 PM (1-hour lunch). No routine evenings or weekends.

Compensation & benefits

• Base salary $162,000-$190,000, commensurate with experience, plus bonus.

Qualifications (required)

• Graduate of an accredited Nurse Practitioner program; eligible for NYSED recognition.
• Current, unrestricted New York State NP and RN licenses.
• AANP or ANCC board certification.
• Current DEA registration.
• Current BLS certification.
• Active CAQH profile and current Medicare and Medicaid provider IDs.
• Proficiency with Microsoft Office and electronic clinical systems.
• Strong clinical judgment, communication, analytical, and organizational skills.

Experience (required/preferred)

• Minimum 2 years clinical experience in primary care, home care, care management, or related settings.
• At least 1 year experience caring for adult patients preferred.

• Bilingual skills.
