Summary:

Leads and serves as a resource for the department. Organizes and conducts medically prescribed physical therapy programs to restore function, prevent disability following disease, injury or loss of a body part, and helps patients reach maximum performance levels. Plans therapy involving physical exercise, massage, heat, water, etc. Utilizes various mechanical and electrical equipment, and prosthetic and orthotic devices. Evaluates, records and reports on patient progress.

Responsibilities:

  • Documents initial evaluations, progress notes, treatments administered, and discharge summaries according to established departmental policies, procedures, and goals in all practice settings as appropriate.
  • Responsible for patient evaluation delivering treatment to referred patients by clinical protocols resulting in patient satisfaction. Notifies physician of evaluation and patient progress as appropriate. Identifies barriers to patient and physician satisfaction and assists with improving entire process as appropriate.
  • Responsible for coordinating and implementing optimal patient care within assigned caseload in an efficient, cost-effective manner. Associate delegates treatments and tasks appropriately and makes suggestions for improvement in skill mix and patient care delivery including demonstrating cross training and flexibility with scheduling.
  • Responsible for charge entry including accurate documentation of treatment and equipment given utilizing appropriate unit/minute guidelines. Enters charges timely on PTR or in computer and turns charges in on a daily basis.
  • Develops, educates, and instructs patient on safe and appropriate exercises during treatment sessions. Provides written home exercise program instruction and ensures patient understanding. Responsible for preparing patient and family for discharge. Makes appropriate plans and recommendations involving continuum of care. Ensures resources are in place prior to patient discharge. Attends patient case conferences and/or completes appropriate paperwork in a timely manner.
  • Responsible for promoting educational growth by attending appropriate educational in-services and
  • Meetings. Contributes to the education of co-workers, students, and/or community by developing or presenting educational materials.
  • Associate performs clinical skills accurately and professionally in assigned area. Associate is recognized for a specialized level of therapy services and assists with program development and marketing to physicians.
  • Associate cleans completely after each patient or delegates task and ensures completion. Associate identifies environment of care concerns and seeks resolution. Associate assists with overall cleanliness of department.
  • Supervises daily performance of staff in assigned work area. Performs counseling's of associates as needed. Completes Associate Performance Feedback Evaluations. Provides input and evaluation of new and transferred associates during their orientation and probationary period.
  • Assists with performance improvement. Monitors documentation and performs chart audits monthly. Completes inventory and maintains appropriate level of supplies and materials in work area. Ensures the delivery of quality services by identifying patient needs, developing quality programs and marketing to physicians.

Requirements:

Education/Skills

  • Demonstrates ability to apply the principles of Collaborative Governance and PI in an organization.
  • Demonstrates ability to work in adverse and variable conditions.

Experience

  • Minimum three (3) years of experience

Licenses, Registrations, or Certifications

  • LPT License in state of employment
  • BLS required.
