We are seeking an experienced Primary Care NP/PA to join our primary care team in Litchfield! We're seeking a compassionate and collaborative primary care Nurse Practitioner or Physician Assistant. You'll have the opportunity to build your own patient panel in a patient-centered environment, with ample support for seamless integration.

- Full-time, Weekdays Only
- Tuesday - Friday schedule
- 4-day work week available if desired
- Outpatient Only
- Adult Medicine (18+)
- No call required

**Role Scope**

- Perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
- Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
- Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

**Requirements**

- Licensure: Unrestricted State of Connecticut Medical License (or eligible)
- Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
- Experience: 2+ years’ experience in a primary care or relevant outpatient setting
