We are seeking an experienced Primary Care NP/PA to join our team in Bristol, Connecticut. This established, multi-provider office features an onsite pharmacist who helps manage complex diabetic and hypertensive patients. You’ll grow as an APP while working alongside an experienced, collaborative team and caring for a diverse patient population.

**Role Scope**

- Perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
- Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
- Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

**Requirements**

- Licensure: Unrestricted State of Connecticut Medical License (or eligible)
- Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
- Experience: 2+ years’ experience in a primary care or relevant outpatient setting
