CHRISTUS Santa Rosa Hospital - Westover Hills (CSRH-WH) is a 253-bed hospital serving the fastest- growing area of San Antonio. Specialized care includes orthopedic and surgical care with advanced spine surgery, Intensive Care, complex neurology for treating stroke and other neurovascular issues, comprehensive cardiovascular care from diagnostics to open heart surgery, vascular lab, sleep center, women’s services, a newborn nursery, the CHRISTUS Weight Loss Institute, wound care, rehabilitation, emergency services, and more. The campus also boasts an Outpatient Imaging Center, an Ambulatory Surgery Center, and four medical plazas, one of which houses our CHRISTUS Santa Rosa Family Medicine Residency Program and CHRISTUS Santa Rosa Family Health Center.
Summary:
The RN Patient Navigator serves as an advocate and patient care coordinator ensuring seamless coordination of services throughout the continuum of care. The RN Patient Navigator serves as a point of contact for physicians, patients, caregivers, and medical institutions to provide resources and assistance with accessing surgical, preventive, diagnostic, therapeutic services, as well as clinical trials offered within CHRISTUS Santa Rosa and in the community. This position is an advocate, coordinator, and facilitator for the patient in their dealings with healthcare providers and organizations throughout the continuum of care. The RN Patient Navigator will offer individualized assistance to help overcome healthcare system barriers. Utilizing the nursing process, the RN Patient Navigator will provide education, evidence-based medical information, and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the surgical care continuum. At all times the RN Patient Navigator will maintain patient confidentiality and comply with HIPAA regulations.
Responsibilities:
- Maintains patient confidentiality and complies with HIPAA standards at all times.
- Guides patients to resources for understanding their diagnosis and treatment options including appropriate clinical research studies and technologies for eligible patients.
- Develops patient education programs and tools.
- Provides navigation services (including education, information, support, and referrals) to the patient and family, helping to make the care seamless, continuous and comprehensive.
- Serves as an essential link between patients and all other care providers. Follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient care management resources.
- Develops an inventory of all surgical services, providers, and institutions, along with contact information, to facilitate coordination of patient care.
- Facilitates appointments for consults and support services within established service standards, including social workers, dietitians, physicians, pastoral care, support groups, etc.
- Coordinates multidisciplinary team planning conferences, develops concise patient summaries for use by the care team, and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
- Attends CHRISTUS service line meetings when requested; Tumor Boards, Surgery Department, and other meetings as necessary to optimize coordination of patient care.
- Coordinates services following completion of individual courses of therapy, developing individualized assessments and Care Plans to assure on-going disease management is maintained and appropriate physician-guided referrals are made.
- Establishes and coordinates a supportive care plan with goals and interventions based upon treatment and individual patient/caregiver needs, preferences, cultural beliefs, and values.
- Identifies barriers to the plan of care and intervenes as appropriate.
- Works within healthcare system and others to collect data, track outcomes, and support strategic planning processes. Delivers report card to Administration, Tumor Board-if applicable- and other groups which demonstrates outcomes and performance improvement activities.
- Adheres to established productivity benchmarks.
- Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, social services staff, etc.).
- Assists with CME compliance and offerings to enhance physician program participation.
- Assists in obtaining and retaining national agency compliance and accreditation requirements: NAP, ACR, ASBS, MQSA, BICOE, CoC, NAPBC.- if applicable.
- Facilitates rapid scheduling/coordination of follow up care, physician-guided multi-disciplinary service referral, compliance and treatment plan.
- Documents all patient encounters in a timely fashion and actively follow-up following appointments to assure treatment plan is being followed and to assist in removing barriers to fulfillment of the Patient Care Plan.
- Determines financial eligibility as may be required for certain grant programs, when applicable, and oversees processes for patient admissions, testing and scheduling of appointments.
- Demonstrates patient service excellence through the distribution of program evaluations. Collect data/metrics to reflect volume and impact program. Evaluate and modifies the program as indicated by evaluation of the data collected. Contributes to an environment of quality and process improvement.
- Efficiently documents medical information using the appropriate forms and/or electronic applications. Maintains and reviews patient records to assure compliance with all grant program requirements.
- Serves as a resource for the community in the area of women’s health. Collaborates and actively participates with local individuals, agencies and organizations to facilitate the increased availability of community-based care services.
- Takes an active role in collaboration with Business Development team to grow service line; identifying new partners in care, providing support to existing partners, and actively seeking opportunities to grow Center.
- Performs other duties as assigned.
Requirements:
Education/Skills
- Associate’s Degree in Nursing required; Bachelor’s degree in Nursing preferred
- Bilingual (English/Spanish) preferred
- Must be able to work in a self-directed environment with the ability to also work with teams
- Requires excellent leadership, problem-solving, decision-making, and critical thinking skills
- Requires excellent communication skills, both oral and written, as well as excellent presentation skills
- Must be computer literate; Microsoft Office competency required
Experience
- Minimum 3 years’ clinical experience required
- Prior experience in Oncology, Surgical Care, and/or women’s health highly preferred
Licenses, Registrations, or Certifications
- Current Texas RN license required
- BLS-Healthcare Provider required
