Requirements

  • Active, valid Licensed Practical Nurse (LPN) state license
  • BLS certification

Core Responsibilities

  • Provide direct nursing care under RN/physician direction: medication administration, wound care, vital signs, and basic treatments.
  • Perform patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
  • Administer prescribed medications and treatments accurately and per protocol.
  • Manage wounds: evaluate, clean, dress, and monitor for infection or complications.
  • Monitor and document vital signs (BP, pulse, temperature, respirations) and changes in condition.
  • Maintain accurate patient records: treatments, medications, observations, and communications.
  • Educate patients and families on conditions, medications, self-care, and preventive measures.
  • Administer and monitor IV medications and fluids; ensure IV line function and site care.
  • Provide catheter care: insertion, maintenance, and monitoring for complications.
  • Collect specimens (blood, urine, sputum) for laboratory testing.
  • Collaborate with RNs, physicians, therapists, and support staff for coordinated care.
  • Attend to patient comfort and safety: positioning, mobility assistance, and pain management.
  • Respond to medical emergencies following facility protocols and provide basic life support as needed.
  • Follow infection control practices, including hand hygiene and PPE use.
  • Handle, store, and dispose of medications and supplies safely and in compliance with regulations.
  • Promote preventive care (vaccinations, screenings) when applicable.
  • Advocate for patient rights and needs within the care team.
  • Supervise CNAs or unlicensed assistive personnel when assigned.
  • Complete other duties as assigned by management.
