Required credentials

• Active Licensed Practical Nurse (LPN) state license.
• Current BLS certification.
• Registry personnel must bring state- or federal-issued photo ID to the facility and wear a photo badge at all times.
• Agency staff must have completed the original or updated COVID-19 vaccination or provide documented medical or religious exemption from their employer. Additional doses must be obtained where required by law.

Key responsibilities

• Provide direct nursing care under RN or physician direction.
• Perform comprehensive patient assessments; administer medications and treatments as prescribed.
• Monitor, document, and report patients’ vital signs and clinical changes.
• Evaluate, clean, dress, and manage wounds; monitor for infection or complications.
• Maintain accurate patient records and educate patients and families on conditions and self-care.
• Respond promptly in medical emergencies and follow infection control protocols.
