Hospice Case Manager (RN) Contract/Travel Grand Junction, CO

Join a contract hospice team in Grand Junction, CO, providing compassionate, patient-centered care Monday–Friday, 8:00 a.m.–5:00 p.m. (days). Competitive pay: $2,004 per 40-hour week. Start: ASAP.

Core responsibilities

• Conduct comprehensive assessments and develop, implement, and evaluate individualized hospice care plans.
• Coordinate care with physicians, nurses, social workers, and other members of the interdisciplinary team.
• Educate and support patients and families about disease progression, symptom management, and end-of-life decisions.
• Maintain accurate, timely documentation that meets hospice standards and regulatory requirements.
• Monitor patient status and revise care plans as clinical needs change.
• Participate in interdisciplinary team meetings and support quality improvement initiatives.

Qualifications

• Active RN license for practice in Colorado (or compact/reciprocal as applicable).
• 1–2+ years nursing experience; hospice, palliative care, or home health preferred.
• Strong clinical assessment skills and sound care-planning judgment.
• Excellent verbal and written communication skills.
• Working knowledge of hospice regulations, standards, and documentation requirements.
