Bilingual Spanish NP or PA – Outpatient Geriatric Primary Care, Brooklyn (Full-Time)

We are hiring a full-time Nurse Practitioner or Physician Assistant for our outpatient clinic in Brooklyn, NY. This role delivers high-quality, person-centered primary care—primarily for Chinese-speaking patients—addressing functional, clinical, and psychosocial needs with an emphasis on independence, wellness, and continuity of care.

Key responsibilities

• Provide comprehensive primary care and preventive services in an outpatient setting.
• Coordinate care and collaborate with a multidisciplinary team to develop and implement individualized care plans and goals.
• Monitor and communicate changes in patients’ clinical status to the care team and facilitate appropriate follow-up.
• Support participants’ functional and psychosocial needs, prioritizing proactive and continuity-driven management.
• Maintain accurate clinical documentation and participate in quality improvement initiatives.

Compensation and benefits

• Salary range: $155,000–$175,000 plus up to 20% performance incentive.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant in New York.
• Board certification required (NP: ANCC or AANP; PA: NCCPA or equivalent).
• Minimum of 2 years’ primary care experience.
• Experience or strong interest in geriatrics preferred.
• Excellent clinical judgment, communication, and organizational skills.
• Bilingual Spanish skills required; experience working with Chinese-speaking patients preferred.
