Chinese-Speaking NP/PA (Locums-to-Perm) – Geriatric Hybrid Primary Care, Brooklyn, $75–$95/hr

Join a value-based primary care team in Brooklyn delivering geriatric-focused care in a hybrid outpatient and home-based model. This locum-to-perm opportunity (conversion expected within 3–6 months) is ideal for an experienced Chinese-speaking Nurse Practitioner or Physician Assistant who wants a predictable schedule, meaningful patient relationships, and a low daily census.

Compensation: $75–$95/hour

Schedule and Patient Volume

• Full-time, Monday–Friday (hours guaranteed)
• Clinic: small daily panel of ~7–10 patients
• Home visits: 3–5 patients per day
• Low daily patient census to allow focused, comprehensive care

Clinical Responsibilities

• Provide full-scope primary care for geriatric patients: preventive care, chronic disease management, and acute care
• Diagnose, prescribe, and develop individualized treatment plans
• Order and interpret labs and imaging; document thoroughly in the EHR
• Deliver patient education and promote wellness for older adults
• Coordinate care with physicians, nurses, and support staff; manage specialist referrals and follow-up
• Maintain compliance with applicable state, federal, and professional regulations

Qualifications

• Active NY NP or PA license
• National board certification (NP or PA)
• Minimum 2 years’ primary care experience, preferably with older adult populations
• Fluent in Chinese (required)
• Medicaid and Medicare provider IDs required
• Expedited credentialing available
