Locum-to-Perm Nurse Practitioner or Physician Assistant — Hybrid Geriatric Primary Care (Queens, NY)

Join a value-driven primary care team focused on caring for older adults in Queens. This locum-to-perm role (conversion expected within 3–6 months) blends outpatient clinic work with home-based visits, giving you variety and the chance to build meaningful patient relationships.

What you’ll do

• Provide full-spectrum primary care for geriatric patients: preventive care, chronic disease management, and acute visits.
• Diagnose, treat, prescribe, and create individualized care plans.
• Order and interpret labs and imaging.
• Document thoroughly in the EHR and ensure timely follow-up.
• Educate patients and promote wellness and prevention.
• Coordinate care with physicians, nurses, and support staff; refer to specialists as needed.
• Maintain compliance with state, federal, and professional regulations.

Work schedule & caseload

• Full time, guaranteed hours
• Monday–Friday (5 days/week)
• Hybrid model: outpatient clinic + home visits
• Home-based care: about 3–5 patients/day
• Clinic visits: approximately 7–10 patients/day
• Low daily census to allow focused, patient-centered care

Compensation

• $75–$95 per hour

Requirements

• Active NP or PA license in New York
• National board certification
• Minimum 2 years’ primary care experience
• Medicaid and Medicare provider IDs required
• Spanish-speaking clinicians preferred
• Expedited credentialing available
