Locums-to-Perm Nurse Practitioner or Physician Assistant — Geriatric Hybrid Primary Care (Bronx)

Join a value-based primary care team serving geriatric patients in The Bronx. This locum-to-perm role (conversion expected within 3–6 months) is a hybrid position combining outpatient clinic coverage with home-based visits.

Compensation: $75–$95/hour

Schedule and Caseload

• Full-time, Monday–Friday (hours guaranteed)
• Clinic: small daily census of 7–10 patients
• Home visits: 3–5 patients per day
• Low overall daily patient load to allow thorough, patient-centered care

Key Responsibilities

• Provide comprehensive primary care for older adults, including preventive care, chronic disease management, and acute problem management
• Diagnose, treat, prescribe, and develop individualized care plans
• Order and interpret diagnostic tests (labs, imaging)
• Document care accurately in the EHR
• Deliver patient education and promote wellness initiatives
• Coordinate care with physicians, nurses, and support staff; manage referrals and follow-ups
• Maintain compliance with state, federal, and professional regulations

Qualifications

• Active NY NP or PA license
• National board certification
• Minimum 2 years’ primary care experience
• Medicaid and Medicare provider IDs required
• Spanish-speaking clinicians preferred
• Expedited credentialing available
