Contract opportunity in Chico, CA for an experienced Registered Nurse to serve as a Patient Care Coordinator. Assignment begins July 1, 2026 for 13 weeks, days shift; pay $2,844 per 40-hour week.

Requirements:

• Current CA RN license and CPR certification
• Minimum 1 year recent acute care case management experience

Role overview:
Coordinate and manage the episode-of-care for an assigned caseload, collaborating with patients, families, physicians, the interdisciplinary care team, and payors to ensure safe, timely, and cost-effective care. Use advanced clinical judgment to assess, plan, implement, and evaluate care; initiate appropriate clinical pathways; and act as a resource for utilization management.

Key responsibilities:

• Coordinate services and transitions from admission through discharge or appropriate transfer
• Serve as patient and family advocate and primary point of contact to facilitate communication among providers
• Optimize resource utilization by applying current clinical knowledge and awareness of community services
• Lead the care team to achieve clinical goals and efficient outcomes
• Evaluate quality and utilization metrics, including admission/discharge criteria, observation/one-day stays, 30-day readmissions, Medicare guidelines, and diagnosis-related criteria
• Participate in ongoing outlier reviews and utilization management activities
• May be assigned to work in the Emergency Department as needed
