Location: Tewksbury, MA
Type: Contract/Travel — 4 weeks
Start: ASAP
Schedule: Days, 8 hrs/day, 40 hrs/week
Rate: Travel $1,468/40 hrs | Local $1,412/40 hrs
Requirements:
- Active LPN license valid in Massachusetts (Tewksbury)
- Minimum 1 year LTC/SNF experience preferred
- Current BLS certification
- Experience with EMRs (e.g., PointClickCare) preferred
- Strong communication, documentation, and teamwork skills
Responsibilities:
- Provide routine nursing care and support residents’ daily health needs
- Administer medications and treatments per orders
- Monitor, assess, and document changes in resident condition
- Assist with ADLs and mobility as required
- Maintain accurate resident records in EMR systems
- Communicate with residents, families, and interdisciplinary team members
- Follow infection control and safety protocols
- Report concerns or changes to RN/supervisor promptly
- Participate in shift handoffs and team meetings
