We are seeking a licensed Physical Therapist for a 14-week contract in Hayward. The role involves evaluating and treating patients across the continuum of care, developing individualized plans, and collaborating with patients and families to optimize functional outcomes.

Requirements:

• Active, valid Physical Therapy state license.
• Current CPR/BLS certification from the American Heart Association or Red Cross (AED included).

Key Responsibilities:

• Perform comprehensive physical assessments and establish evidence-based, patient-centered treatment plans.
• Design and progress individualized therapeutic exercise programs to improve strength, flexibility, endurance, and coordination.
• Deliver hands-on techniques to enhance mobility and reduce pain.
• Utilize therapeutic modalities and recommend appropriate assistive devices and orthotics.
• Provide patient and family education, and maintain clear, timely documentation of interventions and progress.
