Locums-to-Perm NP or PA — Geriatric Primary Care (Hybrid, Bronx/Brooklyn/Ridgewood NY)

Join a value-based primary care team serving geriatric patients in a hybrid outpatient and home-based model. This locum-to-perm opportunity (conversion expected within 3–6 months) offers guaranteed full-time hours and exposure to coordinated, preventive-focused care across Bronx, Brooklyn, and Ridgewood, NY.

Compensation: $75–$95/hour

Schedule and Caseload

• Full-time, Monday–Friday (five days/week) with guaranteed hours
• Low daily census to support comprehensive visits
• Home visits: 3–5 patients/day
• Clinic visits: 7–10 patients/day
• Hybrid setting: outpatient clinic + home-based care in a value-based practice for older adults

Key Responsibilities

• Provide comprehensive primary care for geriatric patients, including preventive care, chronic disease management, and acute care
• Diagnose, treat, prescribe, and develop individualized care plans
• Order and interpret diagnostic tests (labs, imaging) and manage follow-ups/referrals
• Document accurately in the EHR and ensure regulatory compliance
• Deliver patient education and wellness counseling
• Collaborate with physicians, nurses, and support staff to coordinate high-quality care

Qualifications

• Active NY NP or PA license
• National board certification
• Minimum 2 years of primary care experience (geriatric experience preferred)
• Medicaid and Medicare provider IDs required
• Spanish-speaking clinicians preferred
• Expedited credentialing available
