Adult Primary Care Nurse Practitioner or Physician Assistant — Brooklyn, NY (Geriatrics Focus)

We’re looking for a compassionate Nurse Practitioner or Physician Assistant to join our Brooklyn team and provide comprehensive adult primary care with a focus on geriatric patients. This is a full-time, weekday role designed to support high-quality, patient-centered care without nights or weekend shifts.

What the role looks like

• Monday–Friday schedule, no nights or weekends
• Small patient panels for personalized care — typically 8–12 patients/day
• Manageable caseload that lets you spend time on thorough assessments, care planning, and relationship-building with older adults

Compensation and benefits

• Salary: $165,000–$185,000
• 20% production bonus

What we’re looking for

• Licensed NP or PA in New York State
• Minimum 2 years of primary care experience
• Experience in geriatrics preferred, or a strong interest in working with older adults
• Strong clinical judgment, excellent communication, and solid organizational skills
• Medicare and Medicaid ID required
