Locums-to-Perm Nurse Practitioner or Physician Assistant — Hybrid Geriatric Primary Care (Bronx, NY)

Join a value-based primary care practice in The Bronx delivering compassionate, comprehensive care to geriatric patients. This hybrid locum-to-perm role (conversion expected in 3–6 months) blends outpatient clinic shifts with home-based visits and emphasizes preventive care, chronic disease management, and care coordination.

Compensation

• $75–$95 per hour

Schedule & Clinical Model

• Full-time, Monday–Friday (5 days/week); hours guaranteed
• Hybrid: outpatient clinic + home visits
• Low daily census:
  • Clinic: ~7–10 patients/day
  • Home care: ~3–5 patients/day

Key Responsibilities

• Provide full-spectrum primary care for older adults, including preventive services, chronic disease management, acute care, diagnosis, treatment, and medication management
• Order and interpret labs and imaging
• Accurately document care in the EHR
• Deliver patient education and promote wellness initiatives
• Coordinate care with physicians, nursing staff, and specialists; manage referrals and follow-up
• Maintain compliance with state, federal, and professional regulations

Qualifications

• Active NY NP or PA license
• National board certification
• Minimum 2 years of primary care experience
• Medicaid and Medicare provider IDs required
• Spanish-speaking clinicians preferred
• Expedited credentialing available
