Required Qualifications:
- Active RN license
- Minimum 3 years nursing experience
- At least 2 years performing OASIS assessments and developing home health care plans
Primary Responsibilities:
- Provide nursing care to patients in their homes
- Perform OASIS assessments and complete corresponding care plans
- Participate in departmental on-call rotation (one in four nights, includes weekends and holidays)
- Work scheduled daytime shifts, Monday–Friday
Notes:
- Must be able to start on specified date and commit to the 13-week assignment.
