Hospitalist Nurse Practitioner or Physician Assistant — Skilled Nursing (Altoona, WI)

We’re expanding our team that delivers compassionate, patient-centered care to residents in skilled nursing facilities across Altoona, Wisconsin. If you’re a Nurse Practitioner or Physician Assistant who enjoys working with older adults in a collaborative, team-based setting, this could be a great fit.

What the role looks like

• Full-time positions with flexible schedules and six-hour facility shifts (rounds typically start between 7:00 and 10:00 a.m.).
• No overnight shifts or on-call responsibilities.
• Daily rounds to assess and manage acute and chronic conditions, develop treatment plans, prescribe medications, and coordinate care with facility staff and families.
• Efficient documentation supported by a streamlined EHR with AI scribe capabilities, voice dictation, and a company-provided iPad with LTE.
• Access to UpToDate® for immediate clinical guidance.

Basic qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent).
• Prior experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred but not required.
