Job Description

Provide patient-centered primary care to residents in skilled nursing facilities across Cumberland and Florence, WI. This part-time role offers flexible scheduling, no call or overnight duties, and collaborative work with physicians and facility staff to deliver evidence-based care.

Locations

  • Cumberland and Florence, Wisconsin
  • Additional opportunities nationwide: GA, IL, IN, KS, MI, MO, TN

Schedule & Workload

  • Part-time with flexible scheduling
  • Six-hour facility shifts; rounds typically start between 7:00 a.m. and 10:00 a.m.
  • No call or overnight shifts
  • Average 15–20 patient visits per day

Clinical Responsibilities

  • Conduct daily rounds in skilled nursing facilities
  • Manage acute and chronic conditions; assess patients and develop treatment plans
  • Prescribe medications and order/interprete tests as needed
  • Communicate and coordinate care with nursing staff and interdisciplinary teams
  • Ensure compliance with clinical and regulatory standards

Qualifications

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • NP board certification: ANCC, AANPCB, PNCB, or AACN; PA certification: NCCPA
  • Board certified or board eligible in FM, IM, Geriatrics, PM&R, or ED
  • Strong clinical judgment, communication, and patient-care skills
  • Ability to work independently and collaborate within a multidisciplinary team
