Hospitalist Nurse Practitioner / Physician Assistant — Skilled Nursing & Long-Term Care (South Haven, MI)

Location: South Haven, Michigan (additional facility opportunities in Carson City, Charlevoix, Crystal Falls, East Lansing, Escanaba, Iron River, Mt. Pleasant). Additional statewide opportunities in IL, NH, NC, OH, and WI.

Overview:
Provide high-quality, patient-centered care to residents in skilled nursing and post-acute settings. Work as part of a collaborative team to manage acute and chronic conditions in older adults.

Schedule and clinical duties:

• Full-time positions with flexible scheduling.
• Six-hour facility shifts (rounds typically start between 7:00–10:00 a.m.).
• No call or overnight responsibilities.
• Daily rounds, assessment and management of acute and chronic conditions, medication and treatment plan prescribing.
• Efficient documentation using streamlined EHR with AI scribe and voice dictation.

Technology and clinical support:

• Company-provided iPad with LTE and lab coat.
• Access to UpToDate® for clinical guidance.

Requirements:

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA).
• Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred.
