Hospitalist Nurse Practitioner / Physician Assistant — Skilled Nursing & Post-Acute Care (Mt. Pleasant, MI)

Provide high-quality, patient-centered care to residents in skilled nursing and post-acute facilities across Mt. Pleasant, MI, as part of a collaborative, team-based practice focused on older adults.

Locations

• Primary: Mt. Pleasant, Michigan
• Additional Michigan facilities: Carson City, Charlevoix, Crystal Falls, East Lansing, Escanaba, Iron River, South Haven
• Additional states: Illinois, New Hampshire, North Carolina, Ohio, Wisconsin

Schedule & Clinical Duties

• Full-time positions with flexible scheduling
• Typical shifts: six-hour facility rounds beginning between 7:00 a.m. and 10:00 a.m.
• No on-call or overnight responsibilities
• Daily rounds, acute and chronic condition management, create and adjust treatment plans, prescribe medications
• Work collaboratively with interdisciplinary facility teams and support staff

• Streamlined EHR with AI scribe and voice dictation

Qualifications

• Active, unrestricted Nurse Practitioner license
• National certification (e.g., ANCC, AANPCB, PNCB, AACN, or equivalent)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
