- Board certified/eligible in Internal Medicine or Family Medicine
- Eligible for or holding unrestricted Tennessee medical license (free of restrictions, probationary term, or conditions)
- No history of license revocation, restriction, or suspension in any jurisdiction
- Have or willing to obtain unrestricted DEA registration
