Become part of a growing team that delivers compassionate, patient-centered care to residents in skilled nursing facilities across Agawam, Massachusetts. We’re looking for experienced Nurse Practitioners who are committed to improving the health and quality of life of older adults in a collaborative, team-based setting.
Locations and flexibility
- Primary opening in Agawam, MA, with additional opportunities nationwide (IL, MI, NH, NC, OH, WI).
- Full-time roles with flexible scheduling to fit your life.
What your day looks like
- Six-hour facility shifts for full-time clinicians, with rounds typically starting between 7:00–10:00 a.m.
- No overnight shifts or on-call responsibilities.
- Perform daily rounds, manage acute and chronic conditions, develop and prescribe treatment plans, and coordinate care with facility teams.

- Streamlined EHR with AI scribe and voice dictation to minimize documentation burden.
- Company-provided iPad with LTE for easy access on the go.
- Access to UpToDate® for reliable, real-time clinical guidance.
Compensation and benefits

Who we’re looking for
- Active, unrestricted Nurse Practitioner license and national board certification (ANCC, AANPCB, PNCB, or similar).
- Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
- Compassionate, team-oriented clinicians who enjoy working with older adults.
