Job Description

Provide patient-centered primary care to residents in skilled nursing facilities across Cumberland and Florence, Wisconsin. Part-time role with flexible scheduling, no call or overnight responsibilities. Collaborate with physicians, nursing staff, and interdisciplinary teams to deliver evidence-based care.

  • Cumberland and Florence, Wisconsin

Schedule & Clinical Duties

  • Part-time; six-hour facility shifts
  • Daily rounds typically begin between 7:00 a.m. and 10:00 a.m.
  • Average 15–20 patient visits per day
  • Manage acute and chronic conditions, prescribe medications, develop and implement treatment plans
  • Coordinate care with nursing staff and interdisciplinary teams
  • Ensure compliance with clinical and regulatory standards
  • No on-call or overnight shifts

Requirements

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • Board certification (or eligible): NPs — ANCC, AANPCB, PNCB, or AACN; PAs — NCCPA
  • Clinical background in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine preferred
  • Strong clinical judgment, communication, and patient-care skills
  • Ability to work independently and collaboratively within a multidisciplinary team
