Job Description

Provide patient-centered primary care to residents in skilled nursing facilities across Cumberland and Florence, Wisconsin. This part-time role features a flexible schedule with no call or overnight responsibilities and offers collaborative work with physicians and facility staff to deliver evidence-based, high-quality care.

Opportunities also available nationwide in Georgia, Illinois, Indiana, Kansas, Michigan, Missouri, and Tennessee.

Position highlights

  • Part-time with flexible scheduling; typical six-hour facility shifts
  • Rounds generally start between 7:00 a.m. and 10:00 a.m.
  • No on-call or overnight shifts
  • Average 15–20 patient visits per day
  • Perform daily rounds, manage acute and chronic conditions, prescribe medications, and develop individualized treatment plans
  • Coordinate care with nursing staff and interdisciplinary teams
  • Maintain clinical and regulatory compliance
  • Company-provided iPad with LTE

Requirements

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • NP board certification (ANCC, AANPCB, PNCB, or AACN) or PA certification (NCCPA)
  • Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
  • Strong clinical judgment, communication, and patient-care skills
  • Ability to work independently while effectively collaborating within a multidisciplinary team
