Hospitalist Nurse Practitioner or Physician Assistant – Skilled Nursing/Long-Term Care (Troy, OH)

Location: Troy, Ohio

Overview:
Provide high-quality, patient-centered care to residents in skilled nursing and post-acute facilities across Troy, OH. Work collaboratively with interdisciplinary teams to manage acute and chronic conditions, perform daily rounds, and develop treatment and medication plans.

Schedule & Workload:

• Full-time positions with flexible scheduling
• Six-hour facility shifts for full-time providers; rounds typically start between 7:00 a.m. and 10:00 a.m.
• No overnight shifts or on-call responsibilities
• New graduates encouraged to apply

Clinical Tools & Workflow:

• Streamlined EHR with AI scribe and voice dictation

Compensation & Incentives:

Requirements:

• Active, unrestricted Nurse Practitioner license
• Board certification (ANCC, AANPCB, PNCB, AACN, or equivalent)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
