Clinical Lead Nurse Practitioner - Adult Primary Care (TeamCare, NYC)

Join our TeamCare program in New York as a Clinical Lead Nurse Practitioner, providing comprehensive adult primary care while serving as the clinical leader at an assigned site. This role blends direct patient care, care coordination, and leadership responsibilities; you will represent the Medical Director and provide clinical oversight in their absence or at their direction.

Key responsibilities

• Deliver full-scope primary care to adult patients, including assessment, diagnosis, treatment, and care coordination.
• Serve as the clinical lead at an assigned TeamCare location: guide clinical workflow, support staff, and promote quality and safety initiatives.
• Act as the Medical Director’s representative when required, making clinical decisions and supporting practice operations.
• Collaborate with a multidisciplinary team to optimize patient outcomes and continuity of care.
• Maintain accurate clinical documentation and participate in performance improvement activities.

Schedule

• Monday-Friday, 8:30 AM-5:30 PM (1-hour lunch); no routine evenings or weekends.

Compensation & benefits

• Base salary $162,000-$190,000 commensurate with experience, plus bonus potential.

Requirements

• Graduate of an accredited Nurse Practitioner program and eligible for NYSED recognition.
• Current, unrestricted New York NP and RN licenses.
• AANP or ANCC board certification and current DEA registration.
• Active Medicare and Medicaid provider IDs and an active CAQH profile.
• Current BLS certification.
• Minimum 2 years of clinical experience in primary care, home care, care management, or related settings; at least 1 year caring for adult patients preferred.
• Strong clinical judgment, communication, analytical, and organizational skills.
• Proficiency with Microsoft Office and electronic clinical systems/EHRs.
• Bilingual skills a plus.
