Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing & Long-Term Care (DeKalb, IL)

We’re expanding our team that provides high-quality, patient-centered care to residents in skilled nursing facilities across DeKalb, IL. If you’re a compassionate Nurse Practitioner or Physician Assistant who enjoys working with older adults in a collaborative, team-oriented setting, we’d love to hear from you.

What the role looks like

• Full-time positions with flexible scheduling
• Six-hour facility shifts for full-time providers (rounds typically start between 7:00 a.m. and 10:00 a.m.)
• No overnight shifts or on-call duties
• Daily rounds focused on managing acute and chronic conditions, prescribing medications, and developing treatment plans
• Efficient documentation using a streamlined EHR with AI scribe and voice dictation
• Company-provided iPad with LTE for clinical use
• Access to UpToDate® for real-time clinical guidance

What we’re looking for

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred
