Family Medicine Physician - South Bend, IN (Full-Time)

Full-time Family Medicine physician opportunity in South Bend, Indiana. Provide patient-centered primary care for all ages as part of a multidisciplinary team.

Location

• Primary: South Bend, IN
• Additional opportunities: Elkhart and Michigan City, IN

Key responsibilities

• Provide comprehensive primary care across the lifespan
• Diagnose and manage acute and chronic conditions
• Deliver preventive care, wellness exams, and health screenings
• Build long-term patient and family relationships
• Coordinate care with specialists and interdisciplinary team members
• Apply evidence-based medicine to improve outcomes
• Maintain accurate, timely documentation in the electronic medical record

Qualifications

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Family Medicine
• Completion of an accredited Family Medicine residency
• Eligible for or holding an unrestricted Indiana medical license
• DEA certification or eligibility to obtain
• Strong clinical, communication, and interpersonal skills
