Float Family Medicine Nurse Practitioner or Physician Assistant (Abbeville/Greenwood/Laurens/McCormick/Saluda, SC)

We are hiring a Nurse Practitioner or Physician Assistant to serve as a floating provider across outpatient family medicine sites in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties, SC. This role delivers comprehensive primary care with emphasis on prevention, chronic disease management, patient education, and teamwork.

Primary responsibilities

• Provide comprehensive primary care to patients of all ages.
• Travel regularly to assigned family medicine sites across the five counties.
• Deliver evidence-based care focused on wellness, prevention, and chronic condition management.
• Coordinate care with physicians, nurses, and other team members.
• Educate patients and families on diagnoses, treatment plans, medications, and preventive care.
• Accurately and promptly document patient encounters in the electronic health record.

Minimum qualifications

• Graduate of an accredited APRN/NP or PA program.
• Minimum 2 years’ experience in primary care or family medicine.
• Active, unrestricted South Carolina medical license.
• Certified or eligible for certification in specialty (e.g., FNP, PA-C).
