Float Family Medicine Nurse Practitioner or Physician Assistant - Abbeville & Surrounding SC Sites

Seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to serve as a floating primary care provider across outpatient family medicine sites in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties, SC. The provider will deliver comprehensive, evidence-based primary care with emphasis on prevention, chronic disease management, and patient/family education.

Responsibilities

• Provide comprehensive primary care to patients across the lifespan.
• Rotate regularly between assigned family medicine clinics in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties.
• Manage acute and chronic conditions, preventive care, and wellness visits per evidence-based guidelines.
• Collaborate with physicians, nurses, and clinic staff to coordinate patient care and referrals.
• Educate patients and families on diagnoses, treatment plans, medications, and health maintenance.
• Accurately document patient encounters in the electronic health record in a timely manner.
• Maintain clinical competency and comply with clinic protocols and quality initiatives.

Requirements

• Graduate of an accredited APRN/NP or Physician Assistant program.
• Minimum 2 years’ experience in primary care or family medicine.
• Active, unrestricted South Carolina license.
• Certified or eligible for certification in specialty (FNP for NPs; PA-C for PAs).
• Strong communication skills and ability to work independently across multiple clinic locations.
