Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing Facilities (Cahokia & Hillsboro, IL)

Join a growing clinical team delivering patient-centered care to residents of skilled nursing facilities in Cahokia and Hillsboro, IL. We seek compassionate Nurse Practitioners and Physician Assistants dedicated to improving health and quality of life for older adults in a collaborative, team-based setting. New graduates are encouraged to apply.

Locations: Cahokia and Hillsboro, IL. Additional openings nationwide in MA, MI, NH, NC, OH, and WI.

What you’ll do:

• Provide daily rounds in skilled nursing and post-acute care facilities, managing acute and chronic conditions and developing treatment plans
• Prescribe medications and coordinate care with interdisciplinary teams and facility staff
• Work efficient documentation using a streamlined EHR with AI Scribe, voice dictation, and a company-issued iPad with LTE
• Rounds typically start between 7:00 a.m. and 10:00 a.m.; full-time shifts are six hours
• No overnight shifts or on-call responsibilities

Schedule options:

• Full-time and part-time positions with flexible scheduling

Qualifications:

• Active, unrestricted Nurse Practitioner or Physician Assistant license in the U.S.
• Board certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute settings preferred
