Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing Facilities (Wyoming, MI)

Provide patient-centered primary care to residents in skilled nursing facilities across Wyoming, Michigan as a full-time member of a growing healthcare team. This role features flexible scheduling, no call or overnight responsibilities, and close collaboration with physicians and facility staff to deliver high-quality, evidence-based care.

Position highlights

• Full-time with flexible scheduling: six-hour facility shifts; rounds typically start between 7:00 a.m. and 10:00 a.m.
• No call or overnight shifts
• Average 15-20 patient visits per day
• Perform daily rounds, assess and manage acute and chronic conditions, prescribe medications, and develop individualized treatment plans
• Work closely with nursing staff and interdisciplinary teams to coordinate care
• Maintain clinical and regulatory compliance

Requirements

• Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
• NP board certification: ANCC, AANPCB, PNCB, or AACN; PA certification: NCCPA
• Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine preferred
• Strong clinical judgment, excellent communication skills, and commitment to patient-centered care
• Ability to work independently while collaborating effectively within a multidisciplinary team
